Holding your breath underwater

Sometimes when I go underwater I seem to drown at an accelerated rate, probably 2-3x that of normal. When I hit the drowning point it will tick the drowning multiple times every few seconds.

How does this happen? Was the script changed recently? It's been happening to Drogor Splints within the past week, at first I thought it was a DM messing with us somehow, but now I have no clue.
