Any npc that has been engaged in combat in any form seems to get a crazy ability to chase around stealthed players.

They usually do not attack as they can't actually detect you, and normally would not follow you around, but as soon as something triggers them or their associated allies, they just start flocking to stealthed characters and following them around.

Yep same for Invis, AI gets free see invis if you're in the area when it kills something, except it can also start to attack you where as Stealth they will just mob and encircle you.

Not sure quite how to fix it but I've lost PCs over the goofiness through the ages. A fix would be dope.

Just some evidence of how weird / annoying this is

The mongreldog was in combat with some Orcs, then came to me and just followed me forever. Eventually it actually attacked, so I tried to animal empathy it and it assumes the creature actually cannot see me (because it almost assuredly cannot, based on skill checks)

Just a bump - AI pursues things it cannot detect or see after being in combat. I'm also pretty sure this is the effect that makes them hit bleeding targets where in the prior chapters it wouldn't really do that.
