I have died about 6 times and been reduced below 0 probably about a dozen more, yet the transformation effect has never activated.


So I thought I was generally unlucky for a while, but statistically a 1/3 chance should have happened by now.

At subdual/going into negatives it checks once. Then you will have to rest before it checks again. If you have rested inbetween and still see no change report again.

Would it be possible to have the One roll per reset clarified in the perks description? - Though I would personally argue for a short cooldown (several minutes) before another roll if that roll was unsuccessful and have triggering be 1/rest - but thats a suggestion.
