An odd lone pack / fallen adventurers / other packs/corpses - vials dissapear when obtained

On multiple occassions vials I've aquired from these packs and corpses simply dissapear from my inventory upon obtaining them. This time it were Elixirs of Ghosty Visage, but named differently, vials of ... protection. So yeah, perhaps the items spawned inside should just be regular vials of blur, divine shield, etc rather than their own version because they seem to dissapear.

My PC name is Kalannar, if that's even relevant in this case.

[Apocryphal Misconceptions]

If you have a default potion of ghostly visage in your inventory, and then pick up a 'Elixer of the Ghost'' or whatever it's called, then the 'Elixer of the Ghost' will automatically merge with your regular potions of ghostly visage. They're not lost -- but rather just stacked under the default name's version of the vial.

Same goes for the other vials. Could that be what happened?

EDIT:  Potions automatically merging regardless of  having a different name tag will happen in some instances, and won't happen in other instances.

If that is what is supposed to happen, it didn't happen succesfully. They were lost, 100%. There were around 4 exliris in there. I had 4 of my own blur potions (default). When I obtained them, I still only had 4.

And they didn't get replaced, because my default potions were in my quickbar before and after obtaining them. I think it would dissapear from my quickbar if something like that happened.
