Unable to Respawn

On the quest Bones I died twice to a skeleton [Watcher] and was able to respawn. However I died against the boss and it does the normal text for respawn, but it gives me no options but "end dialog"

This may be the problem that occurs if you started as a hardcore prelude PC.

If you die while currently below level 4, even after the prelude, the only option you get is end dialogue at the fugue guy. I had the same problem before. Were you a Hardcore prelude PC to start? If so just get a DM IG and try to stay above level 3.
