Acid Fog poison kills on failing the save a second time

Pretty sure this is a bug, I believe it con drained me to death.

ACID FOG

• SF conjuration increases duration to 1 round / level, GSF Conjuration increases duration to 2 rounds / level.
• SF transmutation lowers fortitude save by -4 for hour/level, GSF transmutation also makes creatures to roll against iron golem fumes poison

No idea what cast the spell, but if poison was involved there, no bug then.
