After using my grapple gun a few times, I noticed I had several duplicates of the "Rope and grapple" In my inventory. I have used the Grapplegun on several pc´s and never seen it happen before. Perhaps it was something to do with lag?

Yes I have noticed that it sometimes happens when I click 2 times on the "recover rope" thing. It is not creating a duplicate everytime however.
