Item Duplicating/Disappearing Bug

From the next reset and if you experience the bug, I want to know the following:

-- What class do you play?
-- The item name?
-- When did you get it?
-- Have you worn it at any point?
-- Have you died while wearing it?
-- Have you been blacked out while wearing it?
-- How many times is it duplicating and when does this happen?

Class: Rogue
Item: Combat Stiletto
Acquired: On Respawn
Equipped at some point: Yes
Killed with it equipped: Never
Blackout-ed with it equipped: Never
Number of times: Two of them duplicated once each, twice now. This happened on Respawn a few hours ago and when raised from the dead three or four days ago.

Class: Rogue
Item: Pyrmancer Piss
Aquired: On a DM quest with Mort, a week or two ago.
Equipped at some point: Yes
Killed with it equipped: No
Blacked out with it: No
Number: One duplicate is found in my pack after respawn. If I die + respawn again, both the duplicate and original piece will duplicate again you and I learned before.

Class: cleric
Item: harpy ring
Acquired: as part of DM loot on several occasions
Equipped: not that I recall
Killed while wearing: no
Blacked out: no
Number: I have four legitimately acquired rings. When I died last night and respawned, I had 8!

I had this happen with two items on the same death.

Class: Bard

Item: Lesser Arcanist's Cloak
Acquired: From one of the Priory's chests two weeks ago
Equipped: No
Killed while wearing: No
Blacked out: No
Number: 2, only once, after respawning

Item: Ooze Hunter's Spear
Acquired: From an Order PC a month ago
Equipped: No
Killed while wearing: No
Blacked out: No
Number: 2, only once, after respawning

-- Has duplicated once, didnt notice until recently.  I have had a respawn in the past few weeks though

-- Arcane Wand of Cure Serious Wounds, 19 charges
-- three days ago, best as I can tell it happened when I leveled up
-- can't be worn
-- neither died nor blacked out
-- so far, only duplicated once.

-- Few days go, to a few weeks ago.
-- Never Worn
-- Never worn while dieing
-- Has died and blacked out.
-- Had two originally, found from different sources, both players, died and they duplicated, Got rid of one of the originals, still get two if I die. Always trash the extras.

-- A couple days ago perhaps is when I got it.

-- It has duplicated once, but only when I consumed all the charges. Once it was consumed the item dissapeared from the ring slot and was back at a random inventory square fully charged.
