Forgotten Forest Transition bugs

There are two broken transitions in the Forgotten Forest.  The transition beside the Mist Impression area leads to a tower thats accessible in an earlier point and you cannot further pass through the Heart of the Forest,

The other is within the Great Canopy in the north-eastern corner.  The transition, when used, leads back on itself.
