Kanntu will not heal wounds per his dialogue.  
 
After you agree to pray to his goddess and then pay his price, he takes the cost for Protection of the Soil (barkskin) instead and then cast it upon you.
 
I tried this twice to ensure I was not misclicking, it happened both times.

I'd like to add if you ask him to cast protection from elements on you, he casts acid fog.

Obviously this is the feature. Samael is not a Paladin, therefore Kaantu must be an evil druid pretending to be a good one under the guise of a "simple" demeanor. Acid Fogging you was intentional, as well as stealing your gold >,>
