Broken Transition Created by Grapple

My PC, Oloorin Graveltoes entered a horizontal climbing area located in the prisoner cell in the Ghaunadauran hideout down the Lower Sewers.

He fired a grapple. However, the clicking on the ensuing transition does nothing. And my PC's stuck on this side of the prisoner cell. Continuing clicking on the transition just gives a bunch of climbing checks and does nothing else and he has no means of retrieving the grapple.

I wonder if a DM can give another "Rope and Grapple" item to my PC? Or at least a refund in coins to get another "Rope and Grapple" is possible? Arc's PC Victor's also present at the scene to testify. Thank you.

Not sure why there was a climbing trigger there, but I removed it.
