Mad Berserker is, as it stands a poor perk. The blindness is absolutely crippling. When fighting someone who is a mad berserker, you can enjoy unlimited sneak attacks, activate stealth to instantly escape (even if you're a non-stealther), or simply stay more than 5 feet from them without difficulty. As devastating as it is to be hit by a Mad Berserker, you honestly aren't going to get hit by them if you have any idea what you're doing.

With most perks, rage is a time where you smile, and go about cleaving goblin heads. Rage for a mad berserker is frustrating where you struggle to find your enemy. One of your class abilities, cleave, even ceases to function while in rage due to the blindness. Given the weirdness of NWN targeting, you'll frequently miss out on  your second (or third, if dual-wielding) attack in the round as one target dies or disappears. You will no longer get attacks of opportunity, making it all the easier to heal from your supposedly devastating attacks.

I understand that the blindness was meant to provide a much needed downside to what would otherwise be an overwhelming perk, but it has crippled rather than balanced it.

On rage, the Mad Berserker should get something more thematically appropriate than simple (and extremely annoying) blindness.

At level 1, Each round they should have to make a will save, and failure means they attack the nearest target, friend or foe. They deal no extra damage, just the normal benefits (+8 STR, -4 WIS, -4 AC, Immunity:Fear, +1d4 Bludgeon Damage, +10% movement speed), as well as an additional -2 to AB.
At level 5, this worsens, as they delve further into their own bloodlust they gain 20% damage vulnerability (causing a net 10% vulnerability) in exchange for the 1d4 bludgeoning damage they would normally get, and they take a -4 to AB and AC.
At level 8, it becomes even worse, as they are utterly consumed by hate. The damage bonus increases to 1d6 but they take a -6 to AB and AC.

At the end of their rage they are affected by ability drain which lasts for 2 minutes, taking some STR, DEX and CON from them.

In short, more penalties that make them more vulnerable to attack, and further lower their AB. But add in more bonuses as they level up and don't cripple them with blindness.
