Replace Disease ON HIT

Well, pretty much it is pretty useless to have these on weapons because the time it takes to even have the disease work, the monster/person will be dead.  I suggest replacing them with poison ON HIT effects.

Poison on hit is typically too good to go on a base weapon that drops on quests.  And there are some weapons with disease on hit which clearly were not meant for monsters, but PCs, or are just meant to give flavor to the concept behind the weapon.

Seems a shame then, since they will almost never be used.  Alas!

Disease is such a lame thing. On PCs it's easily dealt with using a potion to the point they are never actually important, on NPCs they don't do crap since the monster is long dead before it even kicks in.

Diseases should do some instant effect in addition to the disease effect. Maybe -1 con or something.

I agree.  Disease on EFU:A is a joke.  Weapons with it are a joke.  Adding some sort of instant effect would be nice or maybe making them far more deadly/dangerous since you have to wait so long.

The one you get from just wandering around the sewers is pretty nasty if you're a sorc or bard.  Most diseases aren't quite as fast acting and effective though.

That's another kind of disease, but that would be far to strong for a weapon, even epic-dm loot.

Maybe if you pursue this in-game, you could get someone to clean your weapon.

the whole of poison and disease weapons are so you can see the explosion of filth when you succeed, no one cares about the effects but they will be negated by most people immediately anyways

Dunno about that.  In a close fight a bit of con or str damage, or wasting a round negating the effect, could make all the difference.

They aren't mechanically scary, but they're cool.  There's really no reason to replace it.  If it's so weak that it's a non-issue, then it doesn't hurt anyone!

It has a sweet cinematic effect when a grungy ooze cultist or what-have-you can cause all manners of gunk to fly about when he fights.

It does, and I'd say it doesn't bear considering as an actual benefit of the weapon, but more a flavour effect like Light. Unless it's onhit Slaad Eggs or something equally hilarious.
