Okay, so I was just in a mass combat where I died from, near as I can tell, death itself. No spell targeting me, no mob hitting at me, no effect at all. Just... death. How did that happen? O.O

Crim is correct, and I did make a kill command at a bunch of Orogs, so maybe you were tagged by mistake. No bug, just an accident that happens in large-scale events :)
