I was just thinking, you know how you can put water on some maps? Why not put some water into the sewers, like just a shallow pool that goes everywhere in the sewer? I think this would be pretty cool and make it feel more sewer like.

How about a periodic random flow attack of watery sewage that has the capability of a knockdown attack or incur a Reflex save to avoid a 1d6 falling damage? And associate the rushing sound of water to alert those in the path. Or maybe a random water trap where the water suddenly dumps from above in certain spots. A spotted trap would indicate that you hear water coming from above and are forewarned and can act accordingly.
I am doubting this can be made, but thought it would be spicy...

ummmm you do know that rats are VERY good swimmers right? You would be surprised how well those little guys can swim! Well actually rats are not -that- little, some rats are HUGE especially the ones in the Lower Sewers!
Note: Dire Rats are the size of dogs.
At that, having seen the Lower Sewers myself, I should think that everything's fine the way it is, and periodic floodings seem a bit weird, somehow. Water levels won't suddenly rise ten feet, unless something drastic happens, that would be FOIG information.
You only need scub gear in the lower sewers if you are halfling sized :/

Maybe I've got the clunkiest box around, but i can't understand why it has such a drastic effect.

Lower Sewers... *shudders* I agree with Howland, and there would be no reason for random burst of water.
