Critical hits are only scored when a roll of the die falls within a weapon's critical threat. For example, on a rapier the threat range is 18-20, you must roll a natural 18+ to start the critical hit process. Once the first die is cast, a further roll is needed. This is called a threat roll. On this roll, you must hit the target, otherwise the critical hit fails and damage is not multiplied.

hahahaha, ok, i'm showing my age here. There weren't in the first edition rules.....
You just rolled a 20 sided and checked a chart if I remember correctly. Though AC worked entirely different too.... 10 was the worst and the lower it was the better....

There is only one roll in 4e!
Your chance to get a critical is hugely increased if you use truestrike. Especially with things like scimitars and rapiers! At least, that's what I've noticed.
For example, a scimtar 18-20 base crit threat range with true strike, you roll an 18-20 (15% chance to roll the critical threat range) and you roll your second roll with a +20 AB modifyer it is much easier to simply just hit your opponent.
