simply put, the quest would be greatly improved if the Hill was taken away, or the Mob on it was scripted to all come rushing the PCs at once. I've seen this rushing scripted into other quests on Efu, so I know its possible.
Alternatively some could spawn as if they were coming out of buildings, or even teleport.
Anything is better than the current scenario where the mob is scattered about and seemingly stands still even when alchemists fire and fireballs are flung into them, putting the onus on the PCs to have to bodily storm the hill and wage a standing battle where it may be undesirable to do that.
There is also a seemingly empty and unused area within the quest area that seems like a waste of files.

I personally don't care. Nor have ever done the quest. Consider this my standard peanut gallery material and think of it how you like. I just figured the hill would be a logical place for them to want to have you.
Alternatively, you could have a wide open area with little to no places to streamline their masses. Or have them teleport behind the party as well as they attempt to flank them... Who knows. Food for thought.
It does however make sense that an enemy that knows your coming may have a trap set up, or at least preparations made to make you fight them on their terms. In which case you could turn back or attempt to push through with the hopes of slaughtering them all.

I have done this quest numerous times, spiced and unspiced, I believe it is fine as it is. Just because it is difficult to beat, doesn't mean it can't be beaten. It's a difficult quest, and yes, the NPCs have the advantage of being a dug in force. That's the entire idea behind it, a group of lizardfolk who remain in possession of a piece of the Sslal'teesh, fighting to hold it with tooth and nail.
Also, the hill makes entire IC sense too. Highest ground, closer to the heavens, for more praying powah!

The line of sight issues are annoying, but I'm not sure there's an easy fix for this.
