Been thinking about this for a while, and sorry if it's already been suggested (I've checked - couldn't find anything).
What I'd like to suggest is an opportunity to Role Play a character's perma-death. I think it should work something like this.
1. Character dies via PvP or DM or Chooses to end their PC
2. WHen they're in the citadel and they talk the Kelemvor's representative, there can be an option of 'role play your character's death' - this should apply even if death has occured via PvP
3. Character is returned to life briefly (2 mins) and allowed to role-play out an exit from the mortal realm. Perhaps a countdown could appear to the PC or to nearby people as 'XYZ looks a little paler, and thier eyes start to glaze over'
Note - this would permanently kill a character
I think Players would love the chance to role-play their final moments. Imagine the RP possibilities:
* Dying in the arms of your loved one
* Ripping the Orc's Chief's spear from your belly, swearing and annointing your second in command as the new leader of the mercenary band
* Passing on a powerful artefact to a friend
* Telling a carefully-guarded secret on dying
* Confessions of love, or of illicit acts commited by the PC
* Giving you comreades a final goodbye
Imagine the impact it could have on plots...
I think this would not only be a fitting end to some heroic characters (I think only the evil characters who are stoned get to say goodbyes to the game-world) but would be another tool for the great role-players on this server to use.

If you wan to do this, you may be able to get a dm and plan an event around you perma-ing your character. As in, you "script" his death. So that you can get some in-curable disease and slowly wither away for the last month of playing him/her/it. Then in the final moments you can gather your friends and Rp the sick bed guy who writes his will and speaks his last testimonials- then get a DM to perma you. ( I would suggest having the DM around to do this prior to starting your death sequence.
However! Quest and PvP deaths arnt subject to this potential. If you're shot in the head, you die. If you're killed by a PC, they didnt give you time to come back for two minutes and talk some more. >_> I appreciate the story value for character's legitiment demise. So I would suggest the above if you are really through playing a character and want it to be ended in a cool (non-epic) fashion. (Not everyone can sacrifice their life to prevent some great evil from eating the city. )

We do do endings for people if you ask really nicely and have a cool character and we feel so inclined, but i'm not sure we want people coming back, even for 2 minutes.
