I just need a suggestion for what the benifits and strengths of playing one of these would be, the stat adjustments, class, Would it get more movement speed and ac underwater, along with breathing?
Just found this to be a interesting subject, since there is plenty of underwater zones in the underdark ^_^. (might inspire someone to try and play one.)

Pretty much any race that is remotely humanoid can be applied for to play (No pixie half-dragons for LTS). Kuo-toas and likely another 30 or so sub-races that haven't been played, could be played if someone had an awesome enough idea for one.
Kuo-toa would be a tough sell to get approved I would bet, based on the kind of stats they get.
Also, DM's have their own guidelines for what stats and abilities to assign a sub-race. Suggesting what abilities and stats you think a VERY unusual sub-race should get, that you want to play, before you appy for it, might also give DM's the wrong impression about why you want the race.
Just a couple things to keep in mind!

They are scripted into EFU, and they are ECL 3. Due to the incredibly powerful nature of their racial benefits, it's very doubtful anyone will ever be approved for one.

Yeah, all joking aside though, I allowed my brother to play a kuo-toa in PnP once. As a level 3 fighter, he managed to solo an aboleth (keeping in mind he was immune and resistant to most of its nastiness, but a little bit of luck was involved). Considering the power of their racial benefits, we both agreed shortly afterwards it wasn't a good race for our games.
... It would be pwnage if you could get one approved, though.

Wow. I want a Koa-toan friend.
I dont think they would be as likely to get butchered on entry into the city either. >_> Making them more playable then most weird app races. <_<

I wanted to app for a kuo-toan a few months ago, but I knew that without a decent concept, I could forget it.
Now that I see the racial abilities, I think I made the right decision.

Well, they would be awesome to play, and only people who show some skill at rp should play them. Essentailly, ELC could also be a way to show how hard it would be to rp that said creature.
And would they still get underwater breathing?

I did some digging, and I do not know if this is a underdark aquatic race, but I found Sahuagins. From what I can tell, they are somewhat humaniod.
Also found out that Kuo-Toa killing Aboleths makes perfect sense, since they are enemies on par with each other. (Like a orc and elf trying to kill one another, you never know who would win.) . . . or at least, this is what I have summed it up to be. So please tell me if I am wrong.

Technically you could probably play a dragon if you had a super amazing concept! But I'm not sure that'll happen anytime soon either in honesty. This race is incredibly powerful even for its +3 ECL.
Oh and the FR Underdark sourcebook will tell you all you need to know about koa-toans.(Only skimmed it so I can't say for sure about the underwater breathing)
