Are followers of Kelemvor only opposed to people animating the dead, or to the study of negative energy as well (assuming that people are merely using it to blast people and not animating corpses)?
Would the death avatar make sense for a priest of Kelemvor? It summons a shadow, and lets them cast enervation. Would a priest be able to use either and stay in good graces with his deity?

Their opposition is focussed on the animation of the dead, but I imagine they also share a distaste for the use of negative energy, as this is the tool that makes such animation possible.
In our setting, the Death domain does not summon the base Shadow. It will summon varying things depending upon deity, and is only useable at and after level 4. Summoning Kelemvor's, I believe, is not an evil action.

Are there separate titles for high priests and lay priests?

The Church has no official titles for priests. You're welcome to think up your own, though! I have seen 'Doomguard' before.

There aren't that many formal titles for the Kelemvorite church. Doomguide is a special one for a specific PRC, but I find it's always cooler to come up with your own!
