What is the ruling about PvP in places without NPCs, such as the wilderness or hidden areas?

With absolutely no NPCs around, you don't need a DM to initiate it. It is however, better to have one around if you can (we give XP for good PvP etc). All the PvP guidelines must still be followed, as well.
