About logging during combat/bleeding...

That would be a pretty lame thing to do, and I can proudly say I've never done this (my nation of fugue'd characters speaks for itself. :P )

However, I have been in the middle of combat and lost my internet connection or had a local NWN crash. This happened last night, and fortunately it had no negative impact on my character, who was at near-full hit points at time of connection-loss and upon connection-restored -- the combat graciously picked up exactly where it left off. (Though there have been times when this has happened and I've logged back in to discover myself bleeding or in the fugue.)

In light of the DM review of "logging during combat/bleeding," is there a protocol a player is supposed to follow should they crash or their internet connection be lost during-combat/while-bleeding?

The big thing is, make sure you log back on.

To restate it again, at this point in time we are most concerned with characters who log out during the bleeding phase.
