I don't think this is fixable, as it's probably a BioWare or lag issue, but I was out and about and attacked a small groupm of goblins. I thought I'd finished them, but there was a goblin shaman there that I couldn't see. I "tabbed" and the name showed up (although later it didn't show up when I ran). It was casting spells and such, so it couldn't have been the spell "Invisibility".

It was probably working as intended. When you tabbed, you probably heard it, which would show you it's name. You can cast as many defensive magic spells as you want while you are invisible, and a few that are neutral (delayed fireball for example).
It's a bug that sometimes happens.
Definitely a bug, when you pick up a monster through listen it appears blurry. If you can't see it at all (just the name) then you're bugged, re-logging generally fixes it.
Won't DM's call that death-logging; I can just imagine an argument in which the DM tells you you logged for that Stone Lizard/ Orog Tracker whatever, and you said you needed to relog because you couldn't see it, but didn't dare to come back because it would be waiting on you on the place where you logged, getting 5 attacks against a flat-footed you (and obviously killing you with those) before you could react.
I'm sure the DM's wouldnt mind, as long as you told them and not just not mention it.

I don't think I will remember to do such a thing at such a dangerous time; I just hope DM's will trust me on my worth.
I found that sometimes changing zones or any other means to get the critter out of your sight can fix this and make them targettable again as you return to them. So, either summon something as the summons won't ever have this kind of bug, or hope for an attack of opportunity or simply an auto-back-attack on the invisible critter (in which case you would automatically keep attacking, even though you can't click it), use area of effect spells (not recommended..) or simply run, run run away, and live to run another day!
Not everyone has summons. Auto-attacking doesn't occur when you need it, only when you don't need it. And they'll catch up with you on area transfer. Just log out if you can't take their beating.
There seems to be certain sorts of transitions that the AI won't ever be able to use. I always wanted to know if there was a way for builders to influence it, but being one myself I suppose it happens... "at random".. uhm.
So, just try it, fortune favors the bold! :P
Other than that keep those potions of haste/invisibility ready.
Or log out. I just tried giving alternative ways to overcome this problem, and I seldom had problems with those.
I've logged to avoid death to a horde of invisible spiders I couldn't see. I logged in right away with another character, told a DM what happened--and he was completely understanding and even reset the server in case it was a bug on the server end.
If you're always honest with DMs, they're fair.

Areas like the pump house lift cannot be used by monsters. I imagine there may be other types as well. Track transitions purhaps.
The original bug cannot be fixed on our end.
