Quest Bugs:
Duergar's Daughter - 2 quest entries on completion, both dated the same, both saying "You reported back and were rewarded".
Duergar's Erramd - Took this quest while in a party with 3 others. Completed it, we all got both the beginning and the end journal entries, but only I got the rewarded XP.
